Our goal is for each of our patients to achieve a strong, healthy smile in an open, relaxed environment. We respond to our patients' individual needs with quality, personalized care. We have made our new patient forms available to download below. For your convenience and to save time at your first visit with Texas Corner Dental, we recommend completing these forms and bringing them to your first visit.

Please remember to bring your dental insurance card and a form of identification to copy for your records. If you have any problems getting these PDF files to submit or print properly, please call us and we will gladly send or fax them to you.

What you can expect at your Comprehensive Exam Appointment:

  • About an hour for your scheduled appointment
  • We will value your time with punctual seating
  • We welcome early arrival for the first appointment to process paperwork and insurance
  • Gathering of pertinent information
  • Complete exam and decay diagnosis with or without the non-invasive Diagnodent laser
  • Oral Cancer screening
  • Digital X-rays (90% less radiation compared to conventional x-rays)
  • Perio/Gum Tissue analysis to determine the type of cleaning needed
  • One on one discussion in regards to results of your thorough examination and any proposed treatment, complete with written treatment plan

Financial Policy

For your convenience, our office accepts: Cash, Check, Mastercard, Visa, Discover & Care Credit. As a service to our patients, we are pleased to offer the CareCredit financing options. The CareCredit financing program allows you to begin your treatment immediately - then pay for it over time with low monthly payments that are easy to fit into your monthly budget.

Dental Insurance

Our office currently participates with Delta Premier Dental Plan. We also electronically bill most other insurance plans. However, at this point we are not accepting Medicaid Insurance. Our office will use computerized billing to communicate with your insurer; therefore you will not need to bring any insurance forms with you. We will however, require that you bring your insurance card with you on your first visit so that we may have it on record. If there is any question as to whether your particular insurance plan is accepted, please do not hesitate to call our office.
